Important Notes:
1.	A Student must already be admitted to The University of Alabama to apply for an ACM scholarship.
2.	Undergraduate students complete all items below, must have at least a 25 ACT composite score or 1130 SAT composite (math and verbal only), and must have at least a 3.00 overall High School GPA.
3.	Graduate students complete all items except item 8 and item 9.
